How to Join

Becoming a member of Noble AMA IPA is easy.

1. Look in your health plan directory for Noble AMA IPA. The directory will list all of our physicians, their office locations and the health plan code number that you need to use when selecting a physician.
2. Use this number on your health plan enrollment form or when enrolling over the telephone to become a member of Noble AMA IPA. You may also switch to a Noble AMA IPA doctor by contacting your health plan and telling them that you would like to join Noble AMA IPA.
3. Search the online physician directory or call (800) 428-7766 to receive an informational packet about Noble AMA IPA, which includes affiliations, rosters, and other helpful information.
